About this role
Horizon is seeking a compassionate and organized Case Worker to join our Court-Based Mental Health Navigator treatment team to support justice-involved individuals with mental health needs. Navigators will work at various courthouse in Erie County. They will assess mental health and substance use issues of individuals involved in court system to determine appropriate level of care. This role is essential in promoting recovery, reducing recidivism, and improving engagement with treatment.
