Horizon is looking for a Transitional Recovery Support Coach in our Residential facility located in Sanborn, NY. Under the supervision of the Program Directors, the Transitional Recovery Support Coach provides works with individuals specifically enrolled in residential treatment programs and for short periods post- discharge. The goal of the Transitional Recovery Coach is to offer support and resources for individuals as they move through the different stages of treatment assisting with successful linkages to treatment services. The TRC provides educational resources to individuals and supports in the community they can utilize as part of their aftercare. This position is full-time, Monday-Friday 9:00 a.m-5:00 p.m.
