About this role
The Perinatal Navigator will coordinate with a multi-disciplinary provider team in the delivery of comprehensive perinatal services. Responsible for navigating and supporting patients through their obstetric care with focus on several components: psychosocial, nutrition, health education, client orientation, initial assessment, individualized care plan including intervention and reassessments, and postpartum assessment and care. The goal is to improve perinatal access to care and pregnancy outcomes through enhanced prenatal care. Location: This position will work at our Rob Road, Patterson, and Paradise sites. Schedule: Monday – Friday, from 8:00am – 5:00pm. Compensation: $24.18 - $27.06 an hour
