Tabitha Home Health Care and Hospice, LLC - A Collaboration with Immanuel in Omaha, NE is seeking a Social Worker to work PRN/as needed. This schedule includes a weekend and holiday rotation. The Social Worker is responsible for providing direct social services to Seniors/residents/clients and family members to ensure that the medically related emotional and social needs of the Senior/client are met or maintained. This position is also responsible for needs assessment, care planning, problem resolution, discharge planning and client advocacy, and completion of documentation per State and Federal legislations. The Social Worker embraces all components of the job, fostering an environment that allows Tabitha to provide optimum care to Tabitha’s Seniors/clients.
