About this role
Engage clients in a welcoming, hopeful, empathetic manner even during acute phase. Assesses and evaluates the needs of individuals and continues caseload contact to develop goals. Supports individuals in developing appropriate program plans to meet identified needs. Collaborates with Residential Providers and educates provider staff related to Treatment Plans.
