This position is for per diem. JOB SUMMARY Under the intermittent supervision of the Ambulatory Nursing Services Manager, the Ambulatory Nursing RN provides direct and indirect patient care in the ambulatory/primary care setting. Assesses patients, using nursing judgement and appropriate screening protocols to determine unmet healthcare needs. Applies nursing diagnoses to define unmet needs. Organizes interventions to address identified needs, including facilitating referrals, obtaining medical orders, or providing education and case management support for the patient. Effectively communicates identified needs and care plan with the multidisciplinary care team including providers, pharmacists, dieticians, social workers, support groups, and more.
