About this role
OBJECTIVE OF POSITION: To work within a multidisciplinary team to increase the independence and self-efficacy of individuals who are chronically homeless and have SMI and/or co-occurring disorders through outreach support and counseling, direct assistance and training in living skills, advocacy and linkage. By: • Coordinating the care of individuals who are chronically homeless and dually diagnosed including psychiatric/substance abuse, medical, housing, entitlements, financial, legal, social, family, education, vocational, transportation and other activities of daily living.
