RN Care Manager - Providence Child Center, Long Term Care in Portland, Oregon. Work hours are Monday-Friday, 7am-4pm. The Resident Care Manager is accountable for quality of care for children, according to the individualized needs of the child, following the nursing process and in accordance with the Nurse Practice Act. Manages the nursing care of assigned residents, ensuring their maximum independence and self-direction. Coordinates care provision with physicians and other Interdisciplinary Team members in a collegial relationship. Ensures that the resident's care plan is developed, implemented and documented to meet their individual care needs. Accepts professional development as an integral part of nursing practice.
