About this role
The Housing Case Manager is responsible for conducting client assessments and coordinating housing services through the Housing Opportunities for Persons with AIDS (HOPWA) program and other applicable initiatives. This role ensures that Care Resource clients are appropriately evaluated and connected to housing resources, while supporting compliance with program requirements and promoting housing stability. The Housing Case Manager collaborates with internal teams and external partners to facilitate access to housing services and deliver client-centered support aligned with organizational and regulatory standards
