As we continue to grow our team, we are looking for a Case Manager for our ACTT–SW team to join our passionate, dedicated, and “BRAVE” team! If you have case management experience assisting people ages 16 years or older who are experiencing significant difficulties in day-to-day and role functioning as a result of serious and persistent mental illness and/or addictions, then this role is for you!
