About this role
Reporting to the Manager, Health Technology Service Delivery the Health Technology (HT) Coach supports improvements in primary care in the family practice setting while developing quality improvement capacity across practice teams through a tailored practice facilitation approach. The HT Coach is focused on optimizing health technology platforms and workflows (e.g. EMRs, virtual care, clinical decision support tools, etc.) in practice to enable doctors to adopt the attributes of the Patient Medical Home and sustain practice improvements.
