As an LVN, you will provide skilled nursing care, support resident wellness, and collaborate with an interdisciplinary team to ensure the highest quality of care in a supportive post-acute environment. The Opportunity Position: Licensed Vocational Nurse (LVN) – On-Call Location: East Bay Post Acute Pay: $36.00–$40.00 per hour Schedule: Per Diem / On-Call Responsibilities
• Provide direct nursing care in accordance with physician orders and facility policies
• Administer medications and treatments accurately and safely
• Monitor residents' conditions and report changes to physicians and nursing leadership
• Complete timely and accurate clinical documentation
• Assist with admissions, transfers, and discharges
• Collaborate with CNAs, nurses, therapists, and physicians to coordinate resident care
• Ensure compliance with infection control, safety, and regulatory requirements
• Support residents and families with compassion and professionalism
Qualifications
• Current California LVN license in good standing
• Current CPR/BLS certification
• Skilled nursing or post-acute experience preferred, but not required
• Strong clinical assessment and communication skills
• Ability to work independently and as part of a multidisciplinary team
• Reliable, flexible, and committed to providing exceptional resident care
