About this role
_*Shoreline Wellness Centre*_ is a group of mental and physical healthcare professionals with a mission to provide comprehensive client care to people of all ages, utilizing an interdisciplinary approach and responding to the changing needs of the community. As a Registered Social Worker with us, you should have a passion for working with a variety of clients, and be able to implement a variety of modalities to support your clients.
The successful candidate will in our office providing both in person and virtual appointments.
*Responsibilities:*
- Delivering quality counselling services both in-person and virtually, with a focus on in person services.
- Collaborating with members of our team (including, Registered Psychotherapists, Behavioural Therapists, Registered Psychologists, Speech and Language Pathologists, Child and Youth Workers etc) to deliver comprehensive care and service coordination.
- Collaborating with partner organizations as appropriate to address client goals and promote smooth transitions of care.
- Maintaining complete and accurate electronic medical records through our software ?Jane.?
- Actively participating in program planning, quality improvement initiatives and meetings as required
- Communicating effectively with the client and any caregivers, education teams etc as needed, and making sure that a clear plan of care has been communicated to the client.
*Education / Specific Job Requirements:*
- Must be registered and in good standing with the OCSWSSW
- Evidence of professional liability insurance
- Experience working with different types of clients, especially children
- Vulnerable Sector Screen
