*Nurse Practitioner Job Description*
*Overview* We are seeking a dedicated and compassionate Nurse Practitioner to join our Family Medicine practice to work along side Family Physician to cover her practice demands. We are looking for a part time position about 2 days per week. We provide flexibility with the choice of days and hours. We are a great team of 4 Family Physicians with amazing supportive staff. So there is a lot of support available on site.
The ideal candidate will possess strong clinical skills and a commitment to providing high-quality patient care. As a Nurse Practitioner, you will play a crucial role in assessing, diagnosing, and managing patient health issues in Family medicine clinic setting. Your expertise will contribute to improving patient outcomes and enhancing the overall quality of care.
*Duties*
* Conduct comprehensive assessments of patients, including physical examinations and health histories. * Develop and implement individualized care plans based on patient needs and preferences. * Provide ongoing management of chronic conditions, including pain management and dementia care. * Collaborate with interdisciplinary teams to coordinate patient care across various settings. * Utilize electronic health record system. We use PS suite Telus Health. * Educate patients and their families on health maintenance, disease prevention, and treatment options. * Perform laboratory tests and interpret results to guide clinical decision-making. * Ensure compliance with medical terminology standards in documentation and communication.
*Requirements*
* Master?s degree in Nursing from an accredited program. * Current Nurse Practitioner certification and state licensure * Excellent interpersonal skills with the ability to build rapport with patients and families. * Ability to work independently as well as part of a collaborative healthcare team. * Familiarity with electronic health record systems (PS suite) is not mandatory but highly desirable.
