• Join a welcoming interdisciplinary team including physicians, nurse practitioners, nursing staff, and another Social Worker • Work collaboratively to support patients with complex and evolving needs • Contribute to thoughtful, coordinated, team-based care What You'll Do
• Provide counselling and psychosocial support across the lifespan • Support patients with: • Mental health concerns • Addictions • Chronic illness • Life transitions
• Balance both brief, solution-focused support and longer-term therapeutic relationships • Collaborate with team members on care planning and shared decision-making • Participate in case discussions and team meetings • Contribute to the development of programs and services that reflect community needs Why Rural Practice

• Registered with the Ontario College of Social Workers and Social Service Workers • Strong collaboration and teamwork skills • Effective communication skills • Comfortable supporting a wide range of patient needs • Ability to adapt in a dynamic, team-based rural environment
