RN Care Navigator (100% Remote for candidates with a NY State RN license) at Healthmap Solutions. Location: New York or United States. Role: handling calls, educating members, coordinating care Requirements: Active unrestricted RN license required; Bachelor’s degree required (or 6+ years RN experience including 3+ years case management may be considered in lieu). CCM preferred. Case management, dialysis/transplant, and Medicare/Medicaid experience preferred. Proficiency with Compass and Microsoft Excel/OneNote/Outlook/Word.
