We are seeking an experienced Outpatient Hand Therapist with specialized expertise in hand therapy. In this role, you will evaluate and treat patients with upper extremity conditions, including post-operative rehabilitation, helping them regain function, improve mobility, and return to their daily activities with greater independence. As a Certified Hand Therapist or Occupational Therapist, you will develop individualized treatment plans focused on maximizing patient outcomes and enhancing quality of life.
Responsibilities include conducting comprehensive patient evaluations, providing evidence-based therapeutic interventions, educating patients on home exercise programs and self-care techniques, recommending ongoing treatment plans and next steps in care, collaborating with the interdisciplinary rehabilitation team, and maintaining accurate, timely documentation in EPIC. The ideal candidate is passionate about delivering exceptional patient care while promoting functional recovery through compassionate, patient-centered treatment.
A caregiver in this position works Mon-Fri| flexible PRN hours| no weekends or holidays| will cover multiple locations on the West Side–Wellness center in Rocky River, Fairview, Richard E.Jacobs in Avon, Lorain.
A caregiver who excels in this role will:
• Assess patients' ability to safely and effectively perform independent living skills.
• Design the appropriate plan of care, including type and frequency of treatment. Implements the treatment plan using appropriate modalities.
• Document subjective and objective findings of patient progress.
• Monitor and modify the plan of care as needed. Seeks consultation as necessary.
• Educate the patient and family/caregiver about patient deficits.
• Plan and/or assist with patient discharge from therapy services and treatment setting.
• Provide information regarding appropriate selection/use of adaptive equipment and support programs.
• Oversee the care provided by certified occupational therapy assistants and therapy technicians according to established licensing laws, state regulations and OT practice standards.
• Other duties as assigned.
Minimum qualifications for the ideal future caregiver include:
• Bachelor’s Degree
• Graduate of an Occupational Therapy Program accredited by the American Occupational Therapy Association
• Current Ohio licensure as an Occupational Therapist (OT)
• Current Basic Life Support (BLS) certification through the American Heart Association (AHA) OR the American Red Cross

Preferred qualifications for the ideal future caregiver include:
• Experience in a clinical area
• Hand therapy experience
• Certified in Hand therapy
• 3-5 years of Outpatient experience
Physical Requirements:
• Ability to walk without assistive device and to maintain balance despite sudden load in order to support patient from falling. • Ability to meet physical demands characterized at the heavy level defined by 1993 Leonard Matheson and Ministry of Labor. • Ability to lift and reach with arms sufficiently to lift patient's arm and leg during therapeutic treatment program. • Sensation of touch and manual dexterity to be able to palpate muscle tension or to monitor patient pulse. • Visual acuity to observe patient's gross and fine motor movements in order to analyze these movements and to read all patient monitors. • Ability to speak clearly enough to instruct patients and families. • Ability to hear at a conversational level to understand patient's description of symptoms and response to treatment.
