In this role, you'll serve as one of the first points of contact for individuals seeking behavioral health services, conducting assessments, crisis screenings, and connecting clients with the care and resources they need. What You'll Do
• Conduct behavioral health assessments and intake evaluations • Engage with newly referred clients and families within two business days • Develop individualized, person-centered treatment plans • Provide therapeutic interventions, crisis services, and community support • Coordinate care with providers, families, and community resources • Monitor client progress and participate in treatment reviews • Maintain timely, accurate clinical documentation within the electronic health record (EHR)
What We're Looking For Required Qualifications
• Master's degree in Counseling, Social Work, Psychology, Behavioral Health, or a related human services field • Eligible for provisional clinical licensure in Illinois • Minimum of one year of behavioral health or related experience
