Job summary
We are looking for an experienced part-time Physiotherapist to work in GP Practices across Long Lane PCN in Hayes as an MSK First Contact Physio for 25 hours per week on a 12 month fixed term contract. This exciting new role (Band 7) will be joining a growing team that includes Clinical Pharmacists and Social Prescribing Link Workers. Successful candidates will have a named GP Supervisor and access to peer learning developmental opportunities.
The successful candidate will have experience of initial assessments and creating treatment plans. One of our priority focus areas will be around frailty and prevention of falls. Experience in this area would be an advantage.
Main duties of the job
As a First Contact Physiotherapist, the post holder will work as part of a multidisciplinary team across the practices of a Primary Care Network (PCN), with the PCNs GPs and other clinicians and staff, as well as staff and clinicians from community, acute, social and voluntary sectors. They will work directly with patients to assess and treat conditions, manage long-term conditions as well as promote self-care.
The post holder will:
Provide clinical expertise, acting as first-contact physiotherapist and making decisions about the best course of action for patients' care (including in relation to undifferentiated conditions). This will involve seeing patients, without prior contact with their GP, in order to establish a rapid and accurate diagnosis and management plan. Progress and request investigations to facilitate diagnosis and choice of treatment regime, understanding the information limitations derived from these and the relative sensitivity and specificity of particular tests diagnostic services such as x-rays and blood test, and interpret and act on results to aid diagnosis and the management plans of patients. Deliver programmes of supported patient self-management, in ways that facilitate behavioural change, optimise individuals' physical activity, mobility, fulfilment of personal goals and independence, and that minimise the need for pharmacological interventions.

Person Specification
Experience Essential
Minimum of 2 years post-- qualification experience. Experience as Senior Physiotherapist working in autonomous MSK role. Clinical experience of triage and care planning. Understanding of professional ethics/code of conduct and their application in practice Excellent physical examination skills and prioritisation of relevant screening.
Desirable
Knowledge of other specialities common with pain (Rheumatology, Vascular, Endocrinology etc) Clinical experience of triage and care planning Experience of working in Primary care and first contact roles
Qualifications Essential
Right to Work in the UK BSc/Grad Dip in Physiotherapy CPD pertaining to Primary Care and MSK management
Desirable
MSc or MSc modules for advanced practice Evidence of CPD and post registration portfolio CPD pertaining to Primary Care and MSK management Extended Scope competencies around bloods, imaging and prescribing
