Job summary

Millcroft Medical Centre has an exciting oppurtunity for an enthusiastic and proactive pharmacist to join our team.

Main duties of the job

You will become part of the practice Pharmacy Team, being involved in medicines optimisation and have input into the following areas but not limited to: undertaking structured medication reviews, improving medicines optimisation and safety, supporting care homes, and running practice clinics. Pharmacist reviews are expected to reduce avoidable A&E attendances, hospital admissions, avoidable outpatient visits and over-medication.

Within this position you will directly improve patient care and support the efficient running of a new model of general practice.

We are a growing practice with a population of over 30,000 patients working within a purpose built practice in Crewe, Cheshire. We are an EMIS web, DOCMAN, Accurx, paper light practice.

Job description Job responsibilities

To contribute to improving health outcomes in patients with long-term conditions as a member of both the pharmacy team and the wider practice multidisciplinary team.

To provide specialist knowledge and advice on pharmaceutical matters, including cost-effective prescribing, medicines optimisation, and the implementation of clinical guidelines for disease treatment. To also provide evidence-based education and training for colleagues in partnership with the practice.

To practice as an Independent Prescriber, undertaking face-to-face consultations and managing patient care within scope of practice.

To provide advice, guidance, and support for patients presenting with self-limiting conditions.

To work collaboratively to improve communication regarding medication-related issues between the practice and other healthcare providers.

To contribute to the implementation and ongoing development of safe and efficient repeat prescribing systems.

The practice pharmacy team forms part of the wider multidisciplinary team and currently consists of 3 Pharmacists, 2 Pharmacy Technicians, and 3 Prescribing Clerks, all working collaboratively to support safe, effective, and patient-centred care.

Qualifications Essential

Mandatory registration with General Pharmaceutical Council Undergraduate degree in Pharmacy Completed GP training pathway Non Medical Prescribing

Desirable

Membership of the Royal Pharmaceutical Society A member of or working towards Faculty membership of the Royal Pharmaceutical Society

Experience Essential

Minimum of 2 years post-qualification experience In depth therapeutic and clinical knowledge and understanding of principles of evidence-based healthcare An appreciation of the nature of GPs and general practice An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing

Desirable

Produce timely and informative reports
