Job summary
-We are seeking a motivated and experienced Advanced Clinical Practitioner to join to join our busy, friendly, and expanding practice in Yarm.
-The role provides an opportunity for patients to avoid the need to be referred to a GP, thereby enabling them to receive timely care and negate unnecessary delay in receiving treatment.
-An ACP is autonomous in making decisions based on assessment, diagnosis and interpretation of test results and will initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care.
-Full-time or part-time hours considered (34 days per week preferred)

Main duties of the job
The following are the core responsibilities of the Advanced Clinical Practitioner. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
a) Practice in compliance with their respective code of professional conduct and within their scope of practice, being responsible and accountable for their decisions, actions and omissions at this level of practice
b)Support the GP's with home visits, on-call and medical emergencies
c) Demonstrate effective communication skills, supporting people in making decisions, planning care or seeking to make a positive change
d) Use professional expertise and decision-making skills to inform clinical reasoning and manage risk
e) Initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care
f) Manage patients presenting with a range of acute and chronic medical conditions
g) Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health
h) Be a professional role model for the organisation
i)Maintain accurate clinical records in conjunction with extant legislation, using SNOMED CT codes effectively
j) Contribute to practice targets (QOF, etc.), complying with local and regional guidance
k) Support the team with all safeguarding matters in accordance with local and national policies

The Advanced Clinical Practitioner(ACP) has been assessed as competent in practice using their expert clinical knowledge and skills. They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients.
The post holder works at the level of advanced clinical practice and has developed and can evidence the underpinning competencies applicable to the specialty or subject area, i.e., the knowledge, skills and behaviours appropriate to the healthcare professionals (HCP) setting and job role.

Experience Essential
Experience of practice within the four pillars. Job plan that demonstrates advanced clinical practice and has equity with peers working at this level.
Desirable
Experience of working in primary care. Experience of working in a GP Practice. Experience of prescribing and undertaking medication reviews.
Qualifications Essential
Qualified healthcare professional. Educated to Master's degree as detailed within the HEE document "Multi-professional framework for advanced clinical practice in England". Registered on their appropriate professional register.
Desirable
Qualified independent prescriber.
