About this role

Job summary

An exciting opportunity for the Lead Advanced Clinical Practitioner in our Older Persons SDEC Service has arisen, this is a new post.

UHD is currently undergoing huge transformation following investment from the new hospital programme. this will see Poole hospital becoming a planned care site, the largest in the country and the Royal Bournemouth Hospital becoming the emergency /acute care site.

With this investment will see a new OPS SDEC facility being built on the Bournemouth site, enabling the expansion of the existing OPS SDEC service. Promoting an SDEC 1st Philosophy which the successful candidate will lead on the development of, as well as continuously developing the service to so it remains fit for purpose, ensuring timely access, safe quality care for all our patients.

Main duties of the job

To be responsible for the rapid assessment, diagnostics, and treatment of individuals within service using an advanced level of professional accountability, autonomy, and judgement, underpinned by Masters level theory and experience. This includes highly complex decision-making across a broad range of differentiated and undifferentiated individual presentations and complex situations, synthesising information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses.

To provide clinical leadership to the ACP & ECP team and work with others within the directorate to determine the priority workload of the team across the directorate.

To plan and manage complete episodes of care, working in collaboration with the consultants and others, delegating and referring as appropriate, to ensure timely, effective management and optimise health outcomes, in line with the evidence base and nationally recognised best practice.

To play a key operational role and maintain a pro-active approach to managing and leading clinical pathways and flow.

Working closely with a multi-professional team, lead the ACP service in delivery of high quality, performance, and financial frameworks.

To promote and implement service and policy development and redesign, impacting beyond own area and other disciplines, informed by current best practice and evidence-base, in accordance with values-based care of the trust.

Job description Job responsibilities

The post holder will undertake the following in OPS:

Responsible for the leadership, of all enhanced and advanced practice staff, both trainees and qualified within OPS SDEC, as well as supporting educational supervisors and engaging with Higher Education Institutes (HEIs) for the delivery of academic modules.

Oversight of training and development of the ACP team within UHD OPS SDEC.

Maintain appraisals and mandatory training of staff within UHD OPS SDEC via delegated teams.

Responsible for monitoring of risk and governance within UHD OPS SDEC and implementation of any actions

Responsible for local complaints and LERN within OPS SDEC related to the Advancing Practice team.

Use their expert knowledge and complex decision-making skills in unpredictable situations and making a differential diagnosis.

Complete a full system physical examination.

Take a systematic patient history.

Screen patients for early signs of disease and risk factors.

Carry out where appropriate a range of invasive and non-invasive diagnostic and therapeutic procedures to agreed levels of competence.

Order a range of diagnostics tests and procedures.

Make diagnostic decisions based on pathology or imaging results.

Initiate prescribed treatments and medication.

Devise an individual care plan that includes all aspects of multidisciplinary care.

Refer to other health professionals as appropriate.

Discharge patients from hospital / service or arrange for their transfer of care to another health care setting.

Person Specification

Qualifications Essential

Professional registration with relevant professional body. Master's in Advanced Clinical Practice or PGDip as a minimum, must include: 1. History taking and physical assessment. 2. Pharmacology 3. Clinical decision making and diagnostics Supplementary or Independent prescriber Ionising Radiation (Medical Exposure) Regulation trained Evidence of Continuing Professional Development Budget Management Recognised teaching or mentoring qualification ALS provider

Desirable

Recognition as Advanced Practitioner by the National Centre for Advancing Practice Recognised Leadership qualification

Experience Essential

A minimum of five years' experience in relevant specialty. Experience of working in multidisciplinary teams within an acute health care setting Extensive, demonstrable application of advanced clinical skills and experience in relevant specialty. Experience managing, supporting, and supervising staff. including supervision and appraisal of junior staff. Experience of working across organisational boundaries Experience of contributing towards practice or policy developments at local, regional, or national level Evidence of excellent communication skills at all levels with ability to challenge views of multidisciplinary team and present specialist information. Experience of working with a range of internal and external stakeholders. Understanding of the complexities of conducting appropriate research and audit Initiating, developing, and evaluating guidelines and standards. Participation in research and experience of critical appraisal of the evidence base to inform practice. Experience of developing people
