Main duties of the job As a Clinical Pharmacist at Chelston Hall Surgery in Torquay, you will work closely with clinicians to improve patient care and make cost-effective prescribing decisions. You will run telephone triage clinics, assess and manage patients, review pathology results, contact patients, and provide practice-based reviews for those with long-term conditions or complex needs. Additionally, you will handle hospital correspondence related to medicine reconciliation or initiation, liaising with local pharmacies. You're expected to lead the practice repeat prescription service, address queries from reception staff and patients, and participate in home visits and immunisation initiatives. A can-do attitude and excellent communication skills are essential.

Main Responsibilities: As a valued member of the Operose Health team, your key duties will include:
As a member of the practice team, you will work closely with our clinicians to help and improve patient care and make cost effective proscribing decisions. You will run daily telephone triage clinics and call in patients for assessment and management where clinically indicated, as well as review and triage pathology results, contacting patients. Provide practice-based review clinical to those with LYCs or complex health needs. Review and action incoming hospital correspondence, particularly with regards to medicine reconciliation or initiation, liaising with local pharmacies to improve patient outcomes. Lead practice repeat prescription service and deal with queries from reception staff and patients. Qualifications Essential
GPhC registered with excellent communication skills and a team-oriented approach. A willingness to develop clinically and improve patient care.
