Job summary

We are looking for an enthusiastic, experienced and forward-thinking Advanced Clinical Practitioner to join our developing Hospital at Home service.This is an exciting opportunity to work within an integrated community model that brings urgent, complex and proactive care closer to home. Within this model, Urgent Community Response provides the unplanned and urgent response element of Hospital at Home, helping people remain safely at home where clinically appropriate and supporting system flow across the acute and community interface.The role will have a strong focus on frailty, urgent community care, admission avoidance and early supported discharge. It will include working closely with the Community Geriatrician, Nurse Consultants and wider multidisciplinary team to support patients in their own homes, care homes, and other residential care settingsWe are seeking an autonomous practitioner with advanced clinical assessment, diagnostic reasoning and decision-making skills, who is committed to high-quality, person-centred care, service development and improving outcomes for patients in the community.

Main duties of the job

The post holder will provide advanced clinical practice across Hospital at Home, with a particular focus on Urgent Community Response and community frailty pathways.Key duties will include undertaking advanced clinical assessments, formulating differential diagnoses, initiating investigations and developing evidence-based management plans for adults with acute illness, frailty, respiratory conditions, functional decline, deconditioning and complex long-term conditions.The ACP will support rapid response assessments, clinical reviews, treatment planning, personalised care planning, advance care planning and escalation decisions across patients own homes, care homes, residential care settings, Athorpe Lodge and Hospital at Home pathways.The post holder will work alongside the Community Geriatrician and wider multidisciplinary team to support comprehensive geriatric assessment, rehabilitation goals, discharge planning and proactive management of patients with complex needs.The role will also include providing clinical leadership, advice and supervision to colleagues, contributing to admission avoidance, effective discharge, quality improvement, education, audit and service development.

Qualifications Essential

Degree level Qualification Teaching/ Mentorship Qualification Msc in Advanced Clinical Practice or equivalent Evidence of Ongoing Professional Development Portfolio evidencing competency as an ACP

Desirable

Qualified in Non-Medical Prescribing (v300) If applicable for the ACPs Role Current Advanced Life Support Qualification (Essential if Inpatient or UECC ACP, Desirable if Community or non-inpatient ACP)

Registration Essential

Current Professional Registration - NMC/HCPC

Experience Essential

Demonstrate experience in a senior post Experience in multi-disciplinary staff working Experience of working with acutely ill patients Experience of working independently Experience in education of staff Demonstrates
