Job summary

This is an exciting opportunity for a highly motivated Practice Nurse to join our multi-disciplinary practice team.

We are a close working and dynamic team who are passionate about developing and delivering excellent quality local services to meet the needs of our patients.

Our team comprises of 4 GP Partners, 5 Salaried GPs, 3 Practice Nurses, 2 HCAs, 1 First Contact Physio, 1 Paramedic, 1 Clinical Pharmacist, and a supportive and dedicated Administrative Team.

We have a strong culture of learning and staff development and will actively support and encourage development in any areas of special interest.

Main duties of the job

About you :

Practice Nurse preferably with experience working in a General Practice settingExperience in managing diabetes, asthma and COPD is desirable but not essential Looking to work full time and with a desire to work as part of a supportive and enthusiastic team To include work on one Saturday every two months and one weekday evening every two weeks (Extended Access cover)Desirable to have own transport and ability to drive to be able to offer support and care to housebound patients About us :

JOB RESPONSIBILITIES:

Adhere to NMC revalidation requirements and practice within the NMC Code of ConductComply with national infection control standards and health and safety guidelines, working to the IPC PolicyConsult with patients and their families to undertake assessments of patient needSupport in the long-term condition management within the organisation, including being involved with quality improvement and assurance initiativesDevelop, implement and embed an effective care management programme for the frail and housebound patients registered with the organisationSupport public health and screening programmes, including immunisations, vaccinations, and cervical screening and also provide opportunistic health promotion adviceEmbed population health management approaches within general practice, targeting health inequalities and improving accessAs required, provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSFSupport new-to-practice nurses and other healthcare professionals within the organisationTeach and advise patients and their families on how to manage their conditionParticipate in clinical audits and research projects and implements changes as required, including supporting with the development, and updating of practice protocols/guidelines and procedures locallyWork within national and local protocols where these existRecognise the boundaries of their practice and know when and to whom patients should be referredSupport in the delivery of enhanced services and other service requirementsBe aware of duties and responsibilities regarding current legislation and adhere to practice policies and procedures on Safeguarding Adults and Safeguarding ChildrenParticipate in the management of patient complaints when requested to do so and participate in the identification of any necessary learning brought about through incidents and near-miss eventsUndertake all mandatory training and induction programmesContribute to and embrace the spectrum of clinical governanceSupport the practice team with the reviewing and implementation of practice policies and protocols, ensuring conformance to extant legislationAttend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreedProvide immunisations including routine influenza, pneumococcal, shingles and RSV vaccinationsProvide infant immunisationsProvide travel advice and immunisation servicesPerform cervical smears, pregnancy tests and give contraceptive adviceAssist GPs with minor surgeries and coil fittings if requiredCarry out ECGs, venepuncture, urinalysis and wound care (including ulcer/Doppler management and suture removal)Maintain accurate clinical records in conjunction with extant legislation ensuring that SNOMED CT codes are used effectivelyContribute to practice targets (QOF, etc.), complying with relevant guidanceMaintain chronic disease registers and implement an effective call/recall systemPerform practice and housekeeping duties, maintaining a clean, tidy, effective working area at all timesMonitor and order medical stock, drugs and vaccinationsChaperone patients where necessarySupport the team in dealing with clinical emergenciesLiaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)Attend and contribute to practice meetings and training sessionsConduct home visits as requiredThere may be, on occasion, a requirement to carry out other tasks, this will be dependent upon factors such as workload and staffing levels

Person Specification

Experience Essential

Good level of communication skills both verbal and written which can be tailored to a wide range of stakeholders. Good level of ICT knowledge and skills. Good organisational skills and time management. Professional, calm and efficient manner. Self-motivated, can work well independently and within a team. Works safely and effectively when working autonomously and in conjunction with other health care professionals

Desirable

Previous primary care experience working within a GP practice nursing team Experience of NHS IT System EMIS and Docman Experience of clinical audit, evaluation research and implementation of evidence based practice Understanding of national and local guidelines Understand clinical governance and risk management

Knowledge and Skills Essential

Excellent communication skills (written and oral) Strong IT skills Clear, polite telephone manner Competent in the use of Office and Outlook EMIS user skills Effective time management (planning and organising) Ability to work as a team member and autonomously Excellent interpersonal skills Problem solving and analytical skills Ability to follow clinical policy and procedure Experience with clinical risk management

Qualifications Essential

Registered with the NMC and maintains revalidation in line with NMC requirements. Working at Registered Nurse Level Practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework. Enhanced DBS check

Desirable

Hold one or more academic Level 6 diploma or postgraduate certification in long-term conditions care and/or public health initiatives. Evidence of post qualifying and continuing professional development Valid UK driving licence
