About this role

Job summary

Due to expansion within our nursing team we are recruiting for a part time Practice Nurse to join our friendly team at Sonning Common Health Centre.

The vacancy is for 20-30 hours per week, Monday to Friday. Exact working patterns and hours will be discussed in interview. It is a diverse and interesting role working with an established nursing team and wider multidisciplinary team.

This is a varied clinical role for the successful candidate who will undertake a wide range of nursing duties including but not limited to:

Provide high quality evidence-based nursing care

Supporting GP clinics

Childhood and adult immunizations and other injections under Patient Group Directions

Provide wound management including both chronic and post-operative wounds

To carry out Health Checks in conjunction with national local and practice initiatives

Treatment room responsibilities

We seek an individual with excellent communication and IT skills, effective time management and prioritisation skills and with the ability to work well individually and as part of a team.

Main duties of the job

General Treatment room skills

Develop, implement and embed health promotion and well-being programmes

Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE

Provide wound care both post operative and chronic to patients

Provide travel medicine services, within scope of knowledge

Request pathology services as necessary

Process pathology results as required

Maintain accurate clinical records in conjunction with extant legislation

Maintain accurate clinical documentation in accordance with NMC ad Practice Standards

Chaperone patients where necessary

Assist GPs with minor surgery when required

Prioritise health issues and intervene appropriately

Support the team in dealing with clinical emergencies

Implement vaccination programmes for adults and children Under Patient Group Directions.

Liaise with external services and agencies to ensure the patient is supported appropriately - vulnerable patients etc

Delegate clinical responsibilities appropriately, ensuring safe practice and the task is within the scope of practice of the individual

Support the clinical team with all safeguarding matters, in accordance with local and national policies

Deliver opportunistic health promotion where appropriate

About us

The practice is known as Sonning Common Health Centre and is located on Wood Lane, Sonning Common, Reading. The practice was established in the 1950s, the current premises were opened in 1970 and the practice has grown to now care for almost 10,000 patients. From here GP partners and salaried doctors provide high quality, traditional general practice care. The practice falls within the boundary of the NHS Thames Valley Integrated Care Board (ICB). The practice is registered with the Care Quality Commission (CQC) and obtained a report of Outstanding at the most recent review in June 2019.

The practice ethos is that of a strong close-knit practice team, maintaining high standards of patient care in an efficient, well-organised and friendly environment. Patients interests are kept to the fore with a strong emphasis on continuity of care, personal doctoring and integrity in the workplace. A high level of achievement under the Quality and Outcomes Framework (QOF) targets demonstrates this commitment to high quality healthcare.

The partners work together to provide the team with strong and effective strategic management. All partners have a strong commitment to the NHS remaining in public ownership.

The practice has its own dispensary and provides this service for around 30% of its registered population. The practice is an approved training and teaching practice.

Job description Job responsibilities

As a Practice Nurse, you will play an important role in delivering high-quality nursing care as part of a supportive multidisciplinary team. You will provide safe, patient-centred care within your scope of practice, helping to meet the needs of the practice population.

You will be involved in a range of clinical activities, including health promotion and supporting patients to manage their wellbeing. Youll also work closely with the wider clinical team to help deliver, review, and maintain clinical policies and procedures, ensuring care remains effective and up to date.

The following are the core responsibilities of the Practice Nurse. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

In addition to the primary responsibilities, the practice nurse may be requested to:

Monitor and ensure the safe storage, rotation of Medical equipment and dressings.

Participate in local initiatives to enhance service delivery and patient care

Support and participate in shared learning within the practice

Continually review clinical practices, responding to national policies and initiatives where appropriate

Participate in the review of significant and near-miss events applying a structured approach i.e. root cause analysis, RCA.
