We are looking for a experienced, professional, and caring person who can demonstrate highly developed clinical skills with ACP qualifications. We would like and individual who has clinical skills in general practice or urgent care. Our ideal candidate will have experience in a fast paced environment, be confident in their clinical decision making and be able to deliver a high standard of safe clinical care. They should be able to demonstrate critical thinking whilst working within their professional boundaries including independently prescribing appropriate medication and making any necessary referrals for investigations in the appropriate manner.

Main duties of the job

Independently assess, diagnose, and manage a wide range of presentations

Provide safe, evidence based treatment and prescribing

Support same day access and triage where appropriate

Work closely with GPs and the wider MDT to ensure seamless patient care

Participate in clinical governance and quality improvement

About us

Montgomery Medical Practice is a rural, dispensing GP Practice with around 8100 patients situated on the border of Montgomeryshire and Shropshire. Despite its idyllic rural location Montgomery is within a short 30 minute drive of Shrewsbury and an hour and a half from Birmingham. We have a multidisciplinary team that includes Physician Associates, Nurse Prescriber, Practice Nurses and Health Care Assistants.

We are a training practice. We use EMIS Web clinical system and Accurx.

Registered Advanced Nurse Practitioner, ANP)or Advanced Clinical Practitioner, ACP

Independent prescriber

Experience in primary care, urgent care, or a similar setting

Strong clinical reasoning, communication, and organisational skills

A proactive, team focused approach with a passion for patient care

Person Specification

Qualifications Essential

Registered ANP or ACP Independent Prescriber Registration with appropriate professional body

Desirable

Master's degree level in relevant area of expertise

Experience Essential

Clinical assessment skills Excellent communication and teamwork skills Primary care experience Ability to record accurate clinical notes Ability to work within own scope of practice and understanding of when to refer to GPs

Desirable

Ability to effectively triage, assess and diagnose Requesting pathology tests and processing the results, advising patients accordingly Diabetes, Hypertension, Asthma , CHD, Immunisations Understands the importance of evidence-based practice Broad knowledge of clinical governance
