Job summary
Upper Gordon Road Surgery is seeking a motivated and compassionate Nursing Associate to join our friendly and supportive team. The successful candidate will play a key role in delivering high-quality patient care, supporting chronic disease management, and promoting health and wellbeing within our local community and supporting the wider multidisciplinary team.
The post holder is a Nursing associate registered with the Nursing and Midwifery Council.
The post holder will work as part of the general practice nursing team, providing nursing care to patients. The post holder will hold their own clinical caseload to support the needs of patients registered with the Practice and to compliment the needs of the nursing team. This will include undertaking activities as outlined in the current Network Contract and associated direct enhanced services, such as supporting Personalised Care Support Planning and providing cardiovascular health checks.
Working with Multidisciplinary Teams, inclusive of General Practice Nurses and GPs you will deliver collaborative care that focuses on high quality care adding impact and value to benefit patients, and the General Practice Workforce where appropriate. This may include working collaboratively to organise and facilitate care appropriate to the individual needs of the patient.
Main duties of the job
To be able to demonstrate evidenced based practice when undertaking care and delivering Health checks which may be working with patient cohorts who present with i.e., high blood pressure, diabetes, obesity, epilepsy, respiratory issues, constipation, lack of personal hygiene, including foot care
Deliver high standards of nursing care to patientsUndertake chronic disease management including asthma, COPD, diabetes, and hypertension reviewsPerform wound care, dressings, and removal of suturesConduct health promotion and lifestyle advice clinics (e.g. smoking cessation, weight management)Carry out NHS health checks and screening programmesAssist in infection prevention and control, including maintaining clinical standards and audit processesMaintain accurate, contemporaneous patient records in accordance with NHS and practice policiesSupport GPs and the wider multidisciplinary team in delivering patient careParticipate in clinical audits, quality improvement initiatives, and practice meetingsEnsure safe management of clinical stock, vaccines, and equipmentContribute to safeguarding processes for vulnerable adults and childrenTo demonstrate an ability to complete ongoing monitoring, to delegate safely to others and educate with mentoring and supervising others to deliver high quality care templates competently and coherentlyTo demonstrate the ability to carry out a full health assessment employing the support from peers as requiredPhlebotomyVaccination and Immunisations recording

Experience Essential
Understanding of the scope of the role of the Nursing Associate in context of the nursing and interdisciplinary teams. Understands and acts in line with NMC professional standards for practice contained within The Code. Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice. Understand requirements for NMC professional Revalidation. Demonstrate ability to plan and develop care pathways to meet needs holistically for the patient. Experience of providing and receiving complex, sensitive information.
Desirable
Experience of working in General Practice. Experience of managing patients in general practice and supporting patient with learning disabilities, severe mental illness, diabetes and those residing in Care Homes.
Personal Qualities and Attributes Essential
Reliable. Enthusiastic and Self-motivated. Able to work autonomously. Flexible in working arrangements to meet service need. Able to work under pressure to meet deadlines, whilst maintaining accuracy and attention to detail. Able to work at part of a multi-disciplinary team. Commitment to CPD.

Ability to organise and prioritise own delegated workload. Ability to be adaptable, apply resilience and work within own professional scope of practice. Ability to communicate effectively (written, verbal and non-verbal communication) with patients/relatives and carers and all members of the multi -disciplinary team. Ability to develop effective and appropriate relationships with people, their families, carers and colleagues.
Desirable
Ability to support, supervise, assess and act as a role model to nursing associate students , other learners and health care support workers within the SH PCN and clinical setting.
Qualifications Essential
GCSE Maths and English (C and above) or equivalent. Nursing Associate foundation degree qualification. Registered Nursing Associate on the NMC register.
Desirable
Supervision, mentoring, teaching or assessing qualification. Completion of GP fundamentals course
