In this role, you will provide specialist assessment and intervention for individuals with acquired disorders arising from neurological, respiratory and other medical conditions. You will manage a complex caseload that includes residents with tracheostomies, prolonged disorders of consciousness (PDOC), lockedin syndrome and cognitivecommunication disorders, ensuring safe, effective and personcentred care at all times.
You will coordinate and deliver complex communication and dysphagia interventions, using the latest evidencebased approaches to support recovery, quality of life and participation. Working as a key member of the wider multidisciplinary team, you will provide expert advice and clinical leadership around communication and swallowing needs, contributing actively to care planning and decisionmaking.
Alongside your clinical responsibilities, you will play an important role in service development, supporting innovation and continuous improvement within the Speech and Language Therapy service. Key responsibilities:
To provide highly specialist assessment of communication related to clinical conditions which may be complex or compounded in nature, including involvement with the assessment of ‘Prolonged disorders of consciousness’ and ‘Cognitive Communication Disorder’ (CCD). To coordinate assessment, treatment, and outcome measurement for patients with complex Eating, Drinking and Swallowing disorders including utilising Fibre-optic Endoscopic Evaluation of Swallowing (FEES), identifying when to refer for videofluoroscopy of swallow and implementing risk feeding protocols. To provide communication and swallowing assessment and interventions to residents who have tracheostomies and may require ventilator support To utilise clinical reasoning and evidence-based practice to develop and oversee the implementation and evaluation of individualised Speech and Language treatment programmes including writing meaningful goals that are co-produced with the resident, and reviewing outcomes. To utilise a range of verbal and non-verbal communication skills to engage with residents and their families, adapting your communication style to the needs of the resident. To support resident’s communication utilising appropriate low and high tech alternative and augmentative communication (AAC) systems and to engage with local and regional services to ensure (where applicable) residents are able to access appropriate assessment and intervention in this area. To produce reports reflecting highly specialist knowledge regarding resident’s communication and/or swallowing needs, and summarising proposed care plans, goals and outcomes. To provide programmes, advice and leadership to the Therapy Assistants to support the resident's communication and swallowing To carry out specialist level admission assessments to determine a person’s suitability To provide training to the wider MDT as required utilising specialist skills and knowledge To uphold professional standards as outlined by the Royal College of Speech and Language Therapists and the HCPC To be successful in this role, you'll need:
