Job summary

Social Worker B5 Cannock and Seisdon Discharge to Assess Team

An exciting opportunity has arisen for a Social Worker to join our Discharge to Assess team. We are based in Cannock but cover a large geographical area that includes Cannock and surrounding areas up to Seisdon. It is essential therefore that you are a driver and own a car.

The service supports people over the age of 18 to be discharged from hospital with support of a care package and following therapy intervention social care will assess for support to meet identified long term needs. This field of social work is fast paced, and you need to be able to meet short assessment timescales.

Multi-professional meetings are often required, and you will be expected to attend and lead them as required.

Main duties of the job

Experience of working in a multi-professional team is an advantage as we work closely with other health professionals such as Occupational and Physiotherapy, District Nurses, G. P's and Mental health professionals to ensure that the person has a holistic assessment that promotes independence within their own home. ` Knowledge and experience of completing risk assessments is an essential part of the role.

Mental Capacity assessments are frequently required so knowledge of the Mental Capacity Act is essential and experience in this area is preferred

As a qualified social worker your role will require completing Safeguarding enquires and attending any meetings relevant to the investigation including the S42 report.

As a Band 5 Social Worker you will be expected to support Care Act Assessors and Social Care Assessors within their role.

Social Work Learning Academy (SWLA)

On appointment, the post holder will be auto-enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework. The relevant SWLA stage (i.e. stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration. The post holder will be required to take responsibility for their own Continuous Professional Development.

Person Specification

QUALIFICATIONS & TRAINING Essential

Professional Social Work qualification, DipSW, CQSW, CSS, BA, MA or Foreign qualification where certified by the Professional Social Work regulatory body Ensuring current registration with the Professional Social Work regulatory body at all times. An ongoing commitment to personal and professional development. To undergo appropriate training to achieve Assessed Year in Practice (ASYE) Good keyboard / IT skills including word processing, spreadsheets, email and PowerPoint A clear understanding of and commitment to equal opportunities and anti-discriminatory practice

Experience Essential

Experience of working with Adults and older Adults with mental health needs and their carers

Desirable

Experience of social work in other user groups/settings i.e. Children and Families, Learning Disability, Mental Health.
