South Fulham PCN has an exciting opportunity for a GP to work within our thriving PCN.

Main duties of the job
We are seeking a GP to support our PCN and practices. The successful applicant will be expected to work across more than one practice and should be capable of independent travel and the ability to work flexibly to meet PCN and practice needs. The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all patients.
We are keen to attract the right candidate, and will work with you to agree what works best for the practices, PCN, and appointed person we are therefore approaching recruitment with an open mind on the specifics of how the role will function, be it rotating across practices, running a special interest clinic or working alongside our e-hub team.
You will be registered with the General Medical Council (GMC).

Key elements of the role are to:
perform as an autonomous general practitioner working independently - alongside other health care professionals - to assess, diagnose and treat the conditions of patients attending within a primary care settingprovide expert professional advice to patients, carers and colleagues whilst maintaining clinical excellencebe professionally accountable and responsible for all elements of clinical practice in accordance with the GMC code of conduct, ensuring the safety of patients and quality of care You will undertake a variety of duties including surgery consultations, telephone/video consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the practice or external organisations. Assessing the health needs of patients with undifferentiated and undiagnosed problems. Screening patients for disease risk factors and early signs of illness. Contributing to QOF requirements and recording data appropriately. Contributing to PCN requirements and recording data appropriately. In general, the post holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care, as set by Royal College of General Practitioners, General Medical council, all other Royal colleges and councils applicable to the staff within the practice, Department of Health, Clinical Commissioning Groups and all other regulatory and standard setting organisations.
