Job summary
We are looking for an enthusiastic and highly skilled Advanced Clinical Practitioner (ACP) or an Advanced Nurse Practitioner (ANP) to join our friendly, forward thinking general practice team. This is an exciting opportunity to work autonomously within a supportive multidisciplinary environment, delivering high quality, patient centred care to our diverse population.
You will use your advanced clinical skills to assess, diagnose, treat, and manage patients presenting with a wide range of acute, chronic, and complex conditions. The role includes independent prescribing, clinical decision making, and contributing to service development within the practice.
Main duties of the job
The post holder will be an experienced clinician acting within their professional boundaries. They will demonstrate safe, clinical decision-making and expert care for our practice population, from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care.They will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures.

Alton Street Surgery is well established and has a very good reputation for patient care and accessibility. In 2018 the surgery received a rating of Outstanding from the CQC. The practice provides care to approximately 11,700 patients in Ross-on-Wye and surrounding rural areas.
We have a young and enthusiastic Partnership who are family friendly and who encourage a strong team ethos within the surgery. It is very important to the partners that this is maintained and developed. Learning is important in the organisation, and the partners encourage the development of staff.
The practice has the support of a large clinical team, which includes allied health professionals such as an ACP, clinical pharmacists and an experienced nursing and HCA team. We have experienced administration and reception teams who actively manage workflow optimisation, signposting, and pathology delegation, helping to reduce some of the administrative burdens.

The following are the core responsibilities of the ACP / ANP. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
Provide advanced clinical assessment, diagnosis, and management of patientsManage acute presentations, long term conditions, and urgent on the day demandUndertake clinical examinations, investigations, and interpret results to support clinical decisions.Prescribe medications independently in line with current legislation and clinical guidelinesProvide health promotion, disease prevention, and patient educationWork collaboratively with GPs, nurses, pharmacists, HCAs, and the wider PCN team.Contribute to QOF, enhanced services, and practice quality improvement initiativesMaintain accurate, timely clinical records using EMISParticipate in clinical meetings, audits, and governance activities
SECONDARY RESPONSIBLITIES
In addition to these primary responsibilities, the ACP / ANP may also be required to:
Participate in local initiatives to enhance service delivery and patient care Support and participate in shared learning within the organisation Develop an area of specialist interest, taking the lead within the organisation Continually review clinical practices, responding to national policies and initiatives where appropriate Participate in the review of significant and near-miss events applying a structured approach i.e., root cause analysis (RCA) Drive the development of nursing services within the organisation, liaising with external agencies and professional organisations as required Develop practice administrative and clinical protocols in line with the needs of the patient and current legislation

Experience Essential
Proven experience in primary care or urgent/emergency care settings Strong clinical reasoning, decision making, and diagnostic skills Experience of practice within the four pillars Evidence of prescribing and undertaking medication reviews

Qualifications Essential
Registered Nurse or Allied Health Professional with current NMC/HCPC registration. MSc in Advanced Clinical Practice (or working towards completion). Independent Prescriber (V300).

Mentorship or teaching qualification.
