Practice Nurse Chronic Disease Lead (Diabetes Focus, Prescriber Preferred)
Job Overview
Rycroft Primary Care Centre is seeking to recruit an experienced and motivated Practice Nurse to join our clinical team.
We are particularly interested in candidates with a strong background in chronic disease management, especially diabetes care, who are keen to take on a lead role in developing and delivering high-quality diabetes services within the practice.
This is an excellent opportunity for:
An experienced Practice Nurse with independent prescribing qualification, or A nurse with a strong interest in diabetes who wishes to develop advanced clinical and leadership skills, including prescribing
The successful candidate will work as part of a well-established multidisciplinary team and play a key role in improving patient outcomes in long-term conditions.
Main duties of the job
Assess and manage patients nursing needs, including care delegated by GPs Deliver care in line with Practice policies and national guidelines Work autonomously within professional competence and scope of practice Provide a broad range of general nursing care and refer to a GP when appropriate, duties include: wound assessment and management, hypertension and management and immunisations and vaccinations Support and lead chronic disease clinics including:Diabetes, asthma, COPD and cardiovascular disease Take a proactive role in developing and improving diabetes care pathways Monitor, review and optimise patient outcomes in line with QOF and national guidelines Support structured recall systems and long-term condition monitoring Work collaboratively with the clinical team Undertake independent prescribing within scope of practice (if qualified) Support medication optimisation, particularly in diabetes and long-term conditions Work closely with GPs and pharmacists in complex case management Opportunity to undertake prescribing qualification if not already held Cervical screening and new patient health checks Womens health including HRT advice, family planning, breast awareness and swabs Work collaboratively with the wider primary care team to ensure high-quality care Participate in clinical meetings and contribute to service development Support training and development of other nursing staff where appropriate

Our clinical team includes two GP Partners, three Salaried GPs, two Advanced Nurse Practitioners, a Practice Nurse, a Nurse Associate and a Health Care Assistant, who are all supported by our administrative team.

Main Duties of the Role
Assessing Patient Needs
Assess and manage patients nursing needs including care passed to the Nurse by the GP Deliver care in line with Practice Policy and National protocolsGeneral Nursing Care
Provide a broad range of general nursing care and refer to a GP when needed Duties include wound assessment and management pathology samples hypertension management immunisations childhood vaccinations cervical screening and new patient health checks Women's health including HRT family planning breast awareness and swabs health education including smoking alcohol weight diet and medication telephone advice and support chronic disease management
support and deliver chronic disease clinics for diabetes, asthma, COPD, heart disease, spirometry and stroke maintain an appointment and recall system training will be provided to extend clinical skillsLiaison and Teamwork
work with the wider primary care team to ensure high quality care support training and learning for other Practice NursesRecord Keeping
maintain accurate clinical records complete clinical templates in line with QOF and practice standards
Person Specification
Experience Essential
Experience working within a nursing team Experience in chronic disease management Ability to work independently and as part of a multidisciplinary team
Desirable
Experience working in Primary Care Experience managing diabetes clinics or long-term conditions Experience contributing to service development or quality improvement
Qualifications Essential
Registered Nurse with valid NMC registration. Evidence of ongoing professional development and ability to meet revalidation requirements.
Desirable
Independent Prescriber qualification (V300) Recognised training or qualification in diabetes care Additional training relevant to General Practice nursing is desirable.
