Job summary
We are seeking an enthusiastic and motivated Practice Nurse to join our friendly and supportive primary care team. The successful candidate will provide high quality nursing care to our patient population, supporting both acute and long term condition management.
You will work as part of a multidisciplinary team delivering evidence based care, health promotion and screening services. The role includes immunisations, cervical screening, wound care and supporting patients to manage chronic conditions.
We welcome applications from experienced Practice Nurses or nurses with transferable skills who are keen to develop within primary care.

Main duties of the job
The postholder will be responsible for providing evidence-based nursing care including:
Chronic disease management (e.g. asthma, COPD, diabetes)
Cervical cytology
Immunisations and vaccinations (routine and travel where appropriate)
Wound care and dressings
Contraception and sexual health advice (where competent)
Blood pressure monitoring
Phlebotomy
ECG recording
Health promotion and lifestyle advice
Supporting QOF and enhanced services delivery
Maintaining accurate clinical records
Working within practice protocols and NMC guidelines
Scope of practice will be aligned to individual competencies and training.

Job description Job responsibilities Main Duties of the Job
The Practice Nurse will work within their professional competence to provide safe, effective and patient centred care.
Clinical duties
Provide routine nursing care to patients of all ages
Undertake health assessments and screening
Carry out cervical screening in line with national guidance
Administer immunisations and vaccinations according to national schedules and PSDs/PGDs
Provide wound care including dressings, suture and clip removal
Undertake venepuncture, blood pressure monitoring and ECG recordings
Provide NHS Health Checks where required
Provide lifestyle advice including smoking cessation, weight management and healthy living
Support the management of long term conditions including asthma, COPD, diabetes, hypertension and coronary heart disease
Provide contraceptive injections and advice where trained
Chaperone clinicians where appropriate
Identify safeguarding concerns and escalate appropriately
Patient care
Deliver compassionate, patient centred care
Maintain accurate clinical records using the practice clinical system
Maintain patient confidentiality at all times
Support patients to understand and manage their health needs
Provide health education and self management advice

Maintain professional registration and revalidation
Participate in clinical supervision and annual appraisal
Undertake continuing professional development
Work within own competence and seek support when required
Team working
Work collaboratively with GPs, nurses, HCAs and wider practice team

Contribute to audit and quality improvement work

Experience Essential
Experience working as a Practice Nurse, Community Nurse or similar role Experience in chronic disease management Experience delivering immunisations and vaccinations Experience providing wound care and dressings Experience in health promotion and preventative care Experience maintaining accurate clinical records Experience working within clinical protocols and guidelines Experience managing respiratory clinics (asthma/COPD)
Desirable
Experience working as an Independent Prescriber Experience managing diabetes clinics Experience supporting QOF or enhanced services Experience using EMIS Experience working within a multidisciplinary team
Qualifications Essential
Registered Nurse with current NMC registration Chronic disease management training (Asthma/COPD) Evidence of continuing professional development Cervical cytology qualification or willingness to undertake training
Desirable
Independent Prescriber qualification (V300) Immunisation and vaccination training Family planning or sexual health qualification
